Submit evidence of correction to the EHD by <br /> 3/27/2019. <br /> All corrections are due by 3/27/2019. <br /> Notes: <br /> 1) Stericycle is the licensed medical waste hauler for SMP1 and SMP2. Daniel's Sharpsmart is the licensed medical waste <br /> hauler for the surgery center. Medical waste is picked-up once per week by both haulers. <br /> 2) Medical waste generated by the facilities consists of biohazardous waste, chemotherapy waste, consolidated non-RCRA <br /> pharmaceutical and sharps waste, and RCRA pharmaceutical waste. <br /> 3) -RCRA pharmaceutical waste and sharps waste is consolidated into containers which are labeled"Incineration Only." <br /> 4) The small biohazard bags in use in the facility meet the ASTM D1709 and can only be used to collect waste in the <br /> facility; prior to shipping waste off site, the waste in the small bags which only meet the ASTM d1709 must be placed <br /> inside a bag which meets both ASTM standards D1709 and D1922. The large bags in use in the facility meet both the <br /> ASTM D1709 and ASTM D1922 standards. <br />
