HSC117960 Submit an updated copy of the Medical Waste Management Plan to the EHD by 3/19/2020. 24. Consolidated waste is not treated by incineration. The container of consolidated waste is not labeled appropriately. Fe onsolidated waste shall be treated by incineration, and the container shall be labeled with the biohazardous waste symbol and words"HIGH HEAT OR INCINERATION ONLY"on the lid and side so as to be visible from any lateral direction. HSC 118275(a)(7) Some of the consolidated waste containers from Stericycle are not labeled with "HIGH HEAT OR INCINERATION ONLY" on all sides and the lid. The facility is to assure that required labels are on the lids and all four sides prior to placing the containers in the facility. Submit evidence of an in-service training on the labeling of consolidated waste containers for personnel placing the containers in the facility to the EHD by 3/19/2020. 24. Trace chemotherapy waste is not segregated for storage. The secondary container is not labeled appropriately. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately labeled. - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tightfitting cover, and be kept clean and in good repair.. ontainer shall be labeled with the words"Biohazardous Waste"or"BIOHAZARD"on the lid and on the sides so as to be visible from any lateral direction. HSC 118280(c) The biohazard symbol had fallen off the side of the biohazard waste container in urology Room 3219 and required replacing (This violation was corrected during the inspection.)
