Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately labeled. <br /> -Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or"BIOHAZARD"on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c)—The biohazard containers are not labeled on all 4 lateral <br /> sides. The biohazard container located in the cupboard of the Phlebotomy area was uncovered. Ensure all biohazard <br /> containers are labeled on all 4 sides and on the lid. Ensure all biohazard cupboards with biohazard containers are <br /> covered at all times. Provide EHD with photographic evidence of corrections. <br /> Correct all violations and submit evidence of corrections to the EHD <br /> by March 29, 2023. <br /> Notes: <br /> 1. Inspection report emailed to Managers, Michael Martinez (Michael.martinez1atakeda.com and Philomena Miller <br /> (Philomena.westatakeda.com). <br /> 2. The Medical Waste Management Act states all biohazard containers should have a tight fitting lid. The facility's biohazard <br /> containers are contained in secured cupboards with a disposal opening that are covered. This method is approved by the EHD. <br /> 3. EHD was instructed not to take pictures during the inspection. Facility operator will try to obtain permission for future <br /> inspections. <br /> 4. Per section 117630(b) of the Act, the secondary containers shall be lined with a biohazard bag prior to transport from the <br /> facility. <br /> 5. Sharps containers were observed on the Donor Floor that had a wide opening and was uncovered. Per operator, if the tops are <br /> closed after each use then the container will lock and staff will not be able to use them. EHD will further look into sharps container <br /> specification requirements. <br /> 6. Submit an updated Medical Waste Management Plan to the EHD within 30 days. <br /> Reinspecion on/about: A reinspection fee of$156 per hour may be charged. Page 2 of 2 <br /> EH-2/2023 <br />
