The following is an itemized list of underground storage tank violations that have not been addressed for DINO MART as of March 31, 2023. Open violations from January 13, 2023 inspection Violation#209-Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificate dated 10/21/2022 is missing the signature of the designated operator. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#214-Failure to comply with UST operating permit conditions. OBSERVATION:According to the BZ work order found on site, a cold start of the monitoring panel was performed on 4/27/2022 and the EHD was not notified. REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. CORRECTIVE ACTION: Immediately obtain a permit from the EHD. OBSERVATION: The 91 float and chain in UDC 7/8 failed to stop the flow of product at the dispenser when tested. REGULATION GUIDANCE:All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak.
