The following is an itemized list of underground storage tank violations that have not been addressed for DINO MART as of March 31 , 2023 . Open violations from January 13 , 2023 inspection Violation #209 - Failure to maintain a copy of the " Facility Employee Training Certificate " on -site . OBSERVATION : The Facility Employee Training Certificate dated 10/21 /2022 is missing the signature of the designated operator . REGULATION GUIDANCE : All site employees must be trained prior to taking responsibility as a facility employee if after October 1 , 2018 , or within 30 days of hire if prior to October 1 , 2018 . The Facility Employee Training Certificate shall contain accurate information for all employees trained . CORRECTIVE ACTION : Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD . This violation was corrected ❑ This violation will be corrected by ( date ) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #214 - Failure to comply with UST operating permit conditions . OBSERVATION : According to the BZ work order found on site , a cold start of the monitoring panel was performed on 4/27/2022 and the EHD was not notified . REGULATION GUIDANCE : Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed , trained , and certified contractor pursuant to the EHD' s policy and a permit obtained within one business day from the EHD . All other repairs must have a permit submitted to and approved by the EHD prior to beginning work . CORRECTIVE ACTION : Immediately obtain a permit from the EHD . A " Failure to Obtain a Permit" penalty applies . OBSERVATION : The 91 float and chain in UDC 7/8 failed to stop the flow of product at the dispenser when tested . REGULATION GUIDANCE : All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak .
