S A N J 0 A Q I I I Irl Environmental Health Department <br /> C C u N T AMENDED <br /> 4/13/2023 <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> FLYING J TRAVEL PLAZA#617 1 15237 N Thornton Rd, Lodi April 11, 2023 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 206 CCR 2712(i), 2634(e)Failure to submit and have a current UST Response Plan readily available on site. <br /> OBSERVATION:An approved copy of the response plan was not available for inspection. <br /> REGULATION GUIDANCE:A copy of this plan shall be accessible on site at all times. <br /> CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br /> This is a minor violation. <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> 1)OBSERVATION:The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> -The reports indicated that sump sensors were checked but they do not indicate which sumps were opened or <br /> inspected. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> -------------------------- <br /> 2)OBSERVATION:The designated operator failed to check and note the correct test dates for the secondary <br /> containment test on the 11/15/2022 designated operator inspection report. The correct date for secondary <br /> containment testing is 10/28/2022. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> This is a repeat violation, Class Il. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended <br /> and replaces the initial report.
