OBSERVATION: The designated operator failed to check and note the correct test dates for the secondary containment and overfill prevention on the designated operator inspection reports.The secondary containment test date was listed as 3/15/22.The correct date for the secondary containment is 3/29/23. The overfill prevention date was missing in some reports and was listed incorrectly as 3/15/22 in others.The correct overfill prevention date is 5/3/22. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 307 HSC 25290.1(c3), 25290.2(c3) Failure to keep water out of the secondary containment system (post-Jul 2003). OBSERVATION:Water was found in the following locations: -87-grade fill sump. -UDC that contains a black metal cover. REGULATION GUIDANCE: Secondary containment shall be constructed to prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. CORRECTIVE ACTION: The service technician removed 1.5 gallons of liquid from the 87-grade fill sump and returned the sensor to its proper location. Immediately contact a properly licensed,trained, and certified contractor to address the water intrusion into the 87-grade fill sump under permit and inspection of the EHD. This is a repeat violation, Class II. 317 CCR 2636(f)(2) Failure of the line leak detector to detect a 3.0 gph leak at 10 psi and restrict or shut off flow. OBSERVATION: The 91-grade VMI line leak detector failed to detect a leak when tested. REGULATION GUIDANCE:All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. CORRECTIVE ACTION: This was corrected on site. The service technician adjusted the leak detector, retested it, and it passed. This is a Class II violation.
