Ensure that the client consent forms are being completed in full before conducting the body art procedure. <br /> 2. Establish facility procedures to ensure that proof of client age is being kept with the client consent forms. <br /> 19. HSC 119303(a)(5)(B) —(a) Prior to the performance of body art, the client shall read, complete, and sign an informed <br /> consent form that shall include, but not be limited to, all of the following information: (5) Post procedure instructions <br /> that include all of the following: (B) Restrictions on physical activities such as bathing, recreational water activities, <br /> gardening, or contact with animals, and the duration of the restrictions. <br /> OBSERVATIONS: <br /> 1. The post-procedure (aftercare) instructions did not include restrictions on gardening or contact with animals, and the <br /> duration of each of these restrictions. <br /> CORRECTIVE ACTIONS: <br /> 1. Update the aftercare instructions and provide an updated copy to EHD for review. <br /> 37. HSC 119312 (g) & (i)—(g) The health permit shall be posted in a conspicuous place at the body art facility. <br /> Certificates of registration for all practitioners performing body art in that facility shall also be prominently displayed <br /> either near the health permit or at the individual practitioner's procedure area if each practitioner has a designated <br /> area. (i) Health permits shall be renewed annually through a process to be determined by the local enforcement agency. <br /> OBSERVATIONS: <br /> 1. Jorge Ayala's Bloodborne Pathogen certificate was expired. —CORRECTED ON SITE. <br /> 2. Jorge Ayala did not have a posted permit within the facility. Invoice with overdue fees was left at Jorge Ayala's station. <br /> CORRECTIVE ACTIONS: <br /> 1. Have practitioner pay for overdue fees and post permit in a visible area in the facility. Provide proof of correction to EHD. <br /> All corrections due to EHD by 4/18/26. <br /> Notes: <br /> 1. Report is emailed to Andrew Soldana at artisticbinge@gmail.com <br /> 2. Noted to operator to obtain the Sharps Waste Manifests that have the destruction information filled out from the <br /> destination facility. <br /> 3. Facility permit and practitioner permit for Andrew Soldana were observed to be expired on site. Payment has been <br /> received by EHD but permit has not been processed to give to practitioner yet. Noted to the practitioner to send photo of <br /> the new permit posted within the facility. <br /> 4. Reviewed client consent forms from September 2025 through March 9, 2026. <br /> 5. Sterilization certificate was provided for the Cheyenne needle with the lot number 2023-35-097. <br /> 6.
