Page 2 <br />Fly Car Transport, LLC <br />091-12084 <br />DESCRIPTION OF LOSS: <br />It is our understanding that the Fly Car Transport, LLC (FCT) vehicle struck road debris, <br />resulting in damage to the tractor's saddle tank and the release of diesel fuel onto the <br />roadway, paved sidewalk, and into roadside soil. <br />SITE REMEDIATION: <br />Upon receipt of this assignment, we contacted Ancon Marine (Ancon) to provide site <br />remediation. On June 6, 2022, Ancon personnel assessed the site and marked the impacted <br />area for underground utility line locates. <br />On July 14, 2022, Ancon personnel returned to the site and utilized heavy equipment to <br />remove the fuel-impacted soil. Excavation was guided by visual and olfactory observations, <br />as well as a Photo-Ionization Detector (PI D). Once no evidence of spilled fuel remained on <br />site, the area was backfilled with clean fill, graded, and compacted. The restoration of the <br />site was overseen and verbally approved by a City of Stockton representative. <br />WASTE DISPOSAL: <br />A total of 5.40 tons of cleanup-generated waste was transported to the Potrero Hills Landfill, <br />located in Suisun, California. The waste was accepted for disposal on Manifest Number <br />BH1012, on August 31, 2022, by Natasha Saunders. The disposal facility was chosen by <br />Ancon and the disposal documentation is attached to this report. <br />REMARKS: <br />With submittal of this Report, with attachments, we confirm that the remediation is complete <br />and we request that this incident be closed.
