ECEIVED <br />CHIP NARRATIVE 407e <br />Interstate 5, north of Eight Mile Road <br />Collision Report Number 9265-2021-10987 <br />O.E.S. Control Number 21-3855 <br />AUG 1 3 2021 <br />%c:r: <br />Page 2 <br /> <br />1 1. Sequence of Events <br />2 <br /> <br />3 On July 18, 2021, at approximately 2010 hours, Manjeet Singh (Party #1) was driving a 2014 <br /> <br />4 Freightliner three axle tractor and enclosed refrigerator trailer southbound on Interstate 5, just north of <br /> <br />5 Eight Mile Road at a speed in excess of 55 miles per hour. For an undetermined reason, he allowed his <br /> <br />6 vehicle combination to travel off the west roadway edge and down the descending embankment. The <br /> <br />7 vehicle crashed through a wire fence and continued in a southwesterly direction across an alfalfa field. <br /> <br />8 The vehicle combination maintained a high rate of speed as it approached a ravine and raised levee <br /> <br />9 road, which ran perpendicular to Interstate 5. Striking the north edge of the ravine, the vehicle <br /> <br />10 combination became airborne and flew over the ravine and levee road. As the vehicle combination <br /> <br />11 landed in an orchard on the south side of the levee road, the engine oil pan broke open, causing all of <br /> <br />12 the oil to spray onto the dirt. Approximately ten (10) gallons of engine oil was released due to the <br /> <br />13 collision. The oil was absorbed by the dirt in the immediate area of the release and no runoff was <br /> <br />14 observed. The oil contaminated approximately 58 cubic feet of soil. Mr. Singh was the only occupant <br /> <br />15 in the vehicle and no other vehicles were involved. <br />16 <br /> <br />17 2. Environmental impact <br />26 <br /> <br />27 Rich Harreo, of LMH Environmental supervised the cleanup and restoration of this area to its natural <br /> <br />28 state. <br />29 <br /> <br />30 5. Clean-up actions <br />31 <br /> <br />32 The company who owned vehicle # 1 (Doaba Transportation Incorporated) contracted with LMH <br /> <br />33 Environmental to conduct the cleanup and restoration of the scene of the 'spill. The cleanup of the scene <br /> <br />34 under the supervision of The San Joaquin County Environmental Health Services. Fred Earnest, <br /> <br />35 Environmental Health Specialist, of the San Joaquin County Environmental Health Department <br /> <br />36 declared the scene safe at approximately 1330 hours on August 5, 2021. <br />37 <br />Prepared by: S.
