CLIENT NAME. <br /> The following vertsol and/or written instructions were communicated to the client: <br /> 1. Orx:e we put on the wrap, you will need to keep it on a minimum of 24hours. <br /> 2. Keep the area clean and free of any dirt or sun exposure and limit excessive <br /> movements of the area tattooed so that the wrap doesn't loosen and come off. <br /> 3. Keep the area wet when peeling off the wrap, very slowly peel it off from the <br /> top, down and clean the area with Bactine spray. <br /> 4. Once it's off keep the tattooed area clean with antibacterial soap and water, <br /> make sure is dry, then apply a very THIN layer of ointment 1 2 times a day <br /> Either A&D Ointment or Aquophor <br /> 5. Make sure to wash your hands before touching the area, keep the area clean of <br /> any dirt and sweat Keep the tattoo out of the water and sun exposure. <br /> '/o. Apply ointment up until it starts to peel, once it begins Peeling stop 0pplY1n9 <br /> ointment and be careful to not rub anything against it and DO NOT pick at it or peel It <br /> off, that will result rn hosing ink. <br /> 7.Sign a symptoms of infection including but not limited to redness, swelling, <br /> tenderness of the procedure site, red streaks going from ne proce re site towards the <br /> heart, elevated body temperature, or purulent drainage from the procedure si e- for <br /> structrons all a physician it any of the addressed signs and symptoms appear <br /> any other reason related to the Body Art procedure(s) <br /> 8. It physician care is required by the client related to the Body Art procedure(s), the <br /> client is to notify the Body Art facility and practitioner of the problem and the <br /> resolution by a Physician or clinic. This information shall be placed in the client's <br /> file. <br /> 9. Then repeat step 3 to remove the wrap off you can <br /> return to the shop so the artist can place <br /> off from the top down and clean the area with Bactine spray. new wrap on that will <br /> stay on for at least seven more days or you can <br /> leave it off and continue to clean and take care of it. <br /> 4 . Once you have the new wrap on make sure to keep it on for a minimum of seven <br /> days and refer to Step 2 on taking care of it. Then repeat Step 3 to rernove the wrap. <br /> 5_ While the wrap is on, its normal to see a small build up of liquid inside, if possible <br /> leak it out throu he edge of the wrap. If you c:an't, pop a tiny hole in the txrhble to leak <br /> it out and place a new piece of wrap over the area of leal or you can have the artist <br /> do it. <br /> 6 . Signs and symptoms of infection (ncludinq but not limited to lone lasting redness, <br /> swelling tenderness of the procedure site, red streaks going from the procedure site <br /> towards the heart, elevated body temperature, or purulent drainage from the <br /> procedure site. <br /> 7. Call a physician if any of the addressed signs and symptoms appear or for any <br /> other reason related to the Body Art procedure(S). <br /> 8.
