All counter surfaces and service trays in the procedure area shall have a smooth, durable, and <br /> nonabsorbent finish. Remove the absorbent chairs from the procedure area and provide photographic evidence to the EHD. <br /> 35. HSC 119313 -Operation and employee training records present <br /> OBSERVATIONS: <br /> Practitioners did not sign the training record. Practitioner names were written and dated by the same individual. <br /> CORRECTIVE ACTIONS: <br /> Operation and employee training records shall be maintained for 3 years and made available for inspection. A written Infection <br /> Prevention and Control Plan (IPCP) shall be maintained, followed and updated. Ensure the record of training is signed by the <br /> practitioners. Provide a signed copy of the training record to the EHD. <br /> Correct all violations and provide evidence of corrections to the EHD <br /> by May 1, 2026. <br /> Notes: <br /> 1. Inspection report emailed to facility owner, Gerardo Padron, at kumbiaink(a)-gmail.com. <br /> 2. There are two parts to the aftercare instructions. One is a white form and the other is a small pink form. Facility owner is to <br /> ensure clients receive both forms. <br /> 3. The hot water pressure at the restroom sink is low. Facility owner is to ensure there is adequate hot water pressure in the <br /> restroom. <br /> 4. There are currently three practitioners and an apprentice at the facility. <br /> 5.
