FORWARD, INC. - <br /> 5) violation # 5 Section 67104 (b) (d) , Title 22, California <br /> Administrative Code <br /> (b) (1) The owner or operator shall develop and follow a written <br /> schedule for inspecting monitoring equipment, safety and emergency <br /> equipment, security devices and operating and structural equipment (such as <br /> dikes and sump pumps) that are important to preventing, detecting or <br /> responding to environmental or human health hazards. <br /> (2) He shall keep this schedule at the facility. <br /> (3) The schedule shalt identify the types of problems (e.g., <br /> malfunctions or deterioration) which are to be looked for during the <br /> inspection (e.g.. inoperative sunup pump, leaking fitting, eroding dike, <br /> etc.). <br /> (4) The frequency of inspection may vary for the items on the schedule. <br /> However, it should be based on the rate of possible deterioration of the <br /> H equipment and the probability of an environmentat or human health incident <br /> if the deterioration or malfunction or any operator error goes undetected <br /> between inspections. Areas subject to spills, such as loading and <br /> unloading areas, shall be inspected daily when in use. At a minimum,; the <br /> inspection schedule shall include the terms and frequencies required for <br /> permitted facilities under Sections 67244, 67254, 67286, 67343, 67344, <br /> 67403 and 67457, or for interim status facilities under Sections 67244, <br /> 67259, 67314, 67464, 67497 and 67523, where applicable. <br /> (d) The owner or operator shall record inspections in an inspection log <br /> or summary. He shall keep these records for at least three years from the <br /> date of inspection. At a minimum, these records shall include the date and <br /> time of inspection, the name of the inspector, a notation of the observa- <br /> tions made, and the date and nature of any repairs or other remedial <br /> actions. <br /> The facility, has not developed an inspection schedule and <br /> does not maintain an inspection log which includeS the date <br /> and time of inspection, who performed the inspection, ob- <br /> servations made and the date and nature of repairs or reme- <br /> dial actions. <br /> II. Schedule for Correction <br /> These violations were first brought to your attention during <br /> a meeting on January 23 , 1987 . A subsequent letter of Febru- <br /> ary 6, 1987 further informed you of the violations and di- <br /> rected you to take corrective action. Corrective action for <br /> these violations is now past due. <br /> Please send written certification, which contains the sample <br /> certification statement indicated on the enclosed sheet, to this <br /> office as soon as the above corrections have been completed. <br /> The Department will conduct a re-inspection of Forward, Inc. to <br /> verify compliance. <br />
