C <br />MONITORING AND REPORTING PROGRAM <br />FORWARD INCORPORATED <br />WASTE MANAGEMENT FACILITY <br />CLASS II LANDFILLS <br />CLASS III LANDFILLS <br />CLASS II SURFACE IMPOUNDMENT <br />CLASS II LAND TREATMENT UNIT <br />CLOSURE OF CLASS I LANDFILL <br />SAN JOAQUIN COUNTY <br />Cl <br />511 <br />In reporting the monitoring data, the Discharger shall arrange the data in <br />tabular form so that the date, the constituents, the concentrations, and the <br />units are readily discernible. The data shall be summarized in such a manner <br />so as to illustrate clearly the compliance with waste discharge requirements. <br />Monthly monitoring reports shall be submitted to the Board by the 15th day of <br />the month following the month in which the samples were taken. Quarterly, <br />semiannual, and yearly monitoring reports shall be submitted to the Board by the <br />15th day of the month following the calendar quarter in which the samples were <br />taken. <br />The results of any monitoring done more frequently than required at the locations <br />specified herein shall be reported to the Board. <br />A report shall be submitted to the Board by 30 January of each year containing <br />both tabular and graphical summaries of the monitoring data obtained during the <br />previous year. The report shall include a discussion of compliance with the <br />waste discharge requirements. <br />The Discharger shall implement the above monitoring program on the effective date <br />of this Order. <br />Ordered by: <br />WILLIAM H.
