Box 6336 <br /> Stockton, CA 95206 <br /> REPORT OF RECENT INSPECTION AND NOTICE OF VIOLATION, FORWARD <br /> LANDFILL, SAN JOAQUIN COUNTY (CASE NO. 2209) <br /> A copy of an inspection report for your facility is enclosed. The inspection report cites a <br /> number of violations of the landfill's waste discharge requirements(WDRs.). One of these <br /> violations concerns your failure to submit design and construction plans for the landfill <br /> expansion project. We request that you submit the design and construction plans for the <br /> landfill expansion project by 10 November 1988. Construction quality assurance (CQA) <br /> is an integral part of any engineering and construction of a landfill liner. A CQA plan must <br /> be included in the design and construction plans. <br /> The second violation concerns compliance with Monitoring and Reporting Program No. <br /> 88-023. The third quarter monitoring report was due 15 October 1985. We will not take <br /> enforcement action on this violation if the third quarter monitoring report is submitted by <br /> 15 November 1988. <br /> The third violation also involves Monitoring and Reporting Program No.88-023. In order <br /> to comply with the monitoring and reporting program the installation of vadose zone <br /> monitoring equipment is required. Please submit a.time schedule, not to exceed 3 months <br /> in length, detailing the steps you plan to take to achieve compliance with the vadose zone <br /> monitoring requirements. This time schedule is due 15 November 1988.
