Box 388 • Stockton, CA 95201-0388 <br /> (209) 468-3421) <br /> June 6, 1994 <br /> Mr. Jon Whitehill <br /> California Integrated Waste Management Board <br /> 8800 Cal Center Drive <br /> Sacramento, CA 95826 <br /> Re: COVER LETTER- Forward Landfill (#39-AA-001 5) Revised Solid Waste Facilities Permit <br /> San Joaquin County Public Health Services, Environmental Health Division (LEA), certifies that <br /> the Forward Landfill is in conformance with the Public Resources Code, Division 30, Sections <br /> 50000-50002. In addition, the LEA determined that the Forward Landfill Preliminary Closure <br /> and Postclosure Maintenance Plans were complete in a letter to Forward Landfill, Inc. dated <br /> February 7, 1994. Listed below are the enclosures included in this letter: <br /> 1 . The solid waste facilities permit revision application sheet. <br /> 2. The letter from San Joaquin County Public Works Department stating that Forward <br /> Landfill is in conformance with the current County Solid Waste Management Plan. <br /> 3. The Notice of Determination from the San Joaquin County Clerk's office for this <br /> facility's CEOA review. <br /> 4. A copy of the Forward Landfill mitigation monitoring plan. <br /> 5. Preliminary closure and postclosure maintenance plan completeness letters from the <br /> LEA and the California Integrated Waste Management Board. <br /> 6. The finding from the San Joaquin County Planning Commission that this facility is in <br /> compliance with the San Joaquin County General Plan. <br /> 7. The proposed revised permit for the Forward Landfill. <br /> 8. Contaminated Soils Acceptance Program letters from the LEA, dated January 7, 1994; <br /> June 5, 1992; and October 31, 1990. <br /> If you have any questions about the proposed revised permit for Forward Landfill, please call <br /> Greg Oliveira, Senior Registered Environmental Health Specialist, at (209) 468-0341. <br /> Ernest M.
