A body art facilty shall have floors and walls that are smooth, nonabsorbent, free of open holes, and washable. <br /> OBSERVATIONS: <br /> 1. Various holes were found above the practitioner sink. <br /> CORRECTIVE ACTIONS: <br /> 1. Repair the walls to be smooth, nonabsorbent, free of open holes, and easily washable. <br /> Notes: <br /> 1. Report was emailed to erica frese(a)vahoo.com <br /> 2. Chair and blanket made of cloth were observed within the procedure area. Pracitioner removed chair and blanket from <br /> the room. Violation corrected on site. <br /> 3. Viewed client consent forms from 2021-2023. No clients from November 2023-present. Per operator, microblading <br /> services were not being provided. <br /> 4. Pracitioner to add service of tiny tattoos. <br /> 5. Per operator, when doing facials on clients, bedding stays on the procedure bed for comfort. During performance of <br /> permanent makeup, bedding is completely removed and moved to a separate room within the establishment. Bed is then <br /> disinfected with caviwipes and a plastic barrier is placed to cover the procedure bed. <br /> All corrections are due to EHD by March 18th, 2025 <br /> Reinspection on/about: A reinspection fee of$172 per hour may be charged.
