The following is an itemized list of underground storage tank violations that have not been addressed for EMI LS LIQUOR & SPORTS SHOP* as of December 27, 2023. Open violations from May 08, 2023 inspection Violation#208-Failure to comply with the designated operator inspection requirements. OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -Several DO reports do not have the printed name of the UST owner/operator. -Two DO reports do not include a response from the Owner/Operator on Section 6. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#306-Failure to remove,analyze, and properly dispose of water in secondary containment (pre-Jul 2003). OBSERVATION: Liquid was observed in the diesel STP sump. REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. CORRECTIVE ACTION: The service technician removed 2.5 gallons of liquid from the diesel STP sump. Ensure that all sumps and annular spaces are maintained free of liquid. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#308-Failure of the leak detection equipment to have an audible and visual alarm. OBSERVATION: The 91 BRAVO float and chain in the 1/2 UDC failed to stop the flow of product at the dispenser when tested. REGULATION GUIDANCE:All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. CORRECTIVE ACTION: Correct immediately by having a properly licensed,trained, and certified contractor replace the failed component with a functional component(LG 113-x listed, if applicable), and obtain a permit within one business day from the EHD, if needed. If the failed component can't be replaced immediately,there is a possibility that the 91 UST system may be red tagged to prevent fuel inputs. Page 3 of 4
