HSC 119309 - Workstation/procedure area decontaminated <br />OBSERVATION: <br />Piercing Room: <br />Observed sterilized instruments stored on the procedure area counter with non -sterilized instruments. <br />CORRECTIVE ACTION: / <br />Store instruments as described in HSC 119309(a) The practitioner shall maintain a clean and sanitary environment. J <br />32. HSC 119314 - Floors and walls clean and in good repair, adequate light <br />OBSERVATION(S): <br />Permanent Cosmetics Room: <br />1. Rubber cove baseboard is detaching from the wall <br />Piercing Room: <br />2. Flooring threshold is lacking between the procedure room and the sterilization room. <br />3. Observed gaps between on wall surfaces (procedure room wall near sterilization room door). <br />4. Observed gaps in doorframe casing around the sterilization room door. <br />5. The sterilization room -wall and adjoining ceiling dry wall is lined with unfinished wood. <br />6. A piece of vinyl composite tile is missing on the floor in the corner of the procedure area wall near the sterilization room. <br />Tattoo Area: <br />7. Observed a few small holes in on the back gray wall of the procedure area. <br />8. Observed chipped paint on the surface of the small pony wall between the lobby and the procedure area. <br />CORRECTIVE ACTION (S): <br />Permanent Cosmetics Room: <br />1. Adhere rubber base cove to wall <br />Piercing Room: <br />2. Provide threshold or make flooring smooth. <br />3. Seal gaps, provide so floor surface is washable and in good repair. <br />4. Seal gaps, provide so wall surfaces are washable and in good repair. <br />5. Seal gaps, and seal wood provide so wall surfaces are washable and in good repair. <br />6. Repair. Provide so that floor is washable and in good repair. <br />7. Repair/repaint and provide so wall surface is washable and in good repair. <br />8. HSC 119310, 119311 - Jewelry, Inks, Needles etc approved and used correctly. <br />OBSERVATION: <br />Piercing Room: <br />Observed metal jewelry that lacks specifications. <br />CORRECTIVE ACTION: <br />Provide jewelry specifications as seen in section 119310. (b) Only jewelry made of ASTM F138, ISO 5832-1, and AISI 316L or <br />AISI 316LVM implant grade stainless steel, solid 14 -karat through 18 -karat yellow or white gold, niobium, ASTM F 136 6A4V <br />titanium, platinum, or other materials found to be equally biocompatible shall be placed in newly pierced skin. J <br />33.HSC 119314 -Workstation, surfaces, including chairs, armrests, etc. in good repair. <br />OBSERVATION(S): <br />Permanent Cosmetics Room: <br />1 .Absorbent curtain at the entrance. Removed. Corrected onsite. <br />2. Cloth covered abosorbent chairs in the procedure area. Removed corrected onsite. <br />3.
