HSC 119309 - Items washed, disinfected, packaged, labeled, and sterilized <br />OBSERVATIONS: <br />The sterilized instruments were not properly labeled. <br />CORRECTIVE ACTIONS: <br />If not being processed for immediate use, clean instruments shall be placed in sealed sterilization package that contains process <br />indicator, name of instrument, date sterilized, and initial of person/operator. Ensure the name of the instrument (if not <br />identifiable) and the initial of the person/operator is on the package of the sterilized instruments. <br />4. HSC 119315 - Integrators used/monthly spore test/log maintained <br />OBSERVATIONS: <br />1. Spore test results were provided for 2024 and one for 2025. The record provided indicates spore tests were not conducted on <br />a monthly basis and retests were not conducted after a failed spore test. <br />2. The sterilization log does not meet the requirements of the Safe Body Art Act <br />CORRECTIVE ACTIONS: <br />1. Sterilizer shall be spore -tested after initial installation, after major repair, and at least once per month. A written sterilization <br />shall be maintained for 3 years. The log shall include spore -test results each sterilization cycle, date, contents, exposure time <br />and temperature, results of the Class V integrator for every cycle/load, and evidence of an acceptable spore test before reuse of <br />the sterilizer after a failed spore -test. Facility owner is to provide the EHD with monthly spore tests for the next 6 months, <br />beginning April 2025. Facility may not use the autoclave until a passing spore test is provided to the EHD. <br />2. The log shall include spore -test results for each sterilization cycle, date, contents, exposure time and temperature, results of <br />the Class V integrator for every cycle/load, and evidence of an acceptable spore test before reuse of the sterilizer after a failed <br />spore -test. Ensure the contents are documented on the sterilization log. <br />7. HSC 119314 - Sharps containers labeled, used, and disposed of appropriately <br />OBSERVATIONS: <br />1. Facility owner stated sharps waste was disposed by Healthwise on a quarterly basis, but facility owner was unable to provide <br />a manifest(s). <br />2. The sharps waste bin was not properly labeled. <br />CORRECTIVE ACTIONS: <br />1. Sharp waste containers shall be disposed by a licensed waste hauler or approved mail back system. Documentation of proper <br />disposal shall be maintained for 3 years. Provide EHD with evidence of proper sharps waste disposal. <br />2. The sharps waste container shall be within arm's reach and labeled with the word "sharps waste" or with the biohazard <br />symbol and the word "Biohazard". Label the sharps waste bin as indicated above and provide photographic evidence of <br />correction to the EHD. <br />13. HSC 119307 - Bloodborne Pathogen training <br />OBSERVATIONS: <br />The current Bloodborne Pathogen Training Certificate was not provided for Katrina Bambula-Santos. <br />Reinspection on/about: A reinspection fee of $172 per hour may be charged.
