HSC 119315 - Integrators used/monthly spore test/log maintained <br />OBSERVATIONS: <br />The following results were provided for the spore tests for 2025: <br />January 14 <br />passed <br />February 6 <br />serial # doesn't match; passed <br />March 13 <br />expired product, unable to process <br />April 12 <br />serial # doesn't match; failed <br />May 6 <br />passed <br />May 21 <br />passed <br />Even though the spore tests were conducted on a monthly basis there were issues with the serial number of the sterilizer not <br />matching or the test failed. Operator has not provided a spore test for June 2025 or July 2025. <br />CORRECTIVE ACTIONS: <br />Sterilizer shall be spore -tested after initial installation, after major repair, and at least once per month. A written sterilization shall <br />be maintained for 3 years. The log shall include spore -test results each sterilization cycle, date, contents, exposure time and <br />temperature, results of the Class V integrator for every cycle/load, and evidence of an acceptable spore test before reuse of the <br />sterilizer after a failed spore -test. Operator is to spore test the sterilizer each month and provide results of the test to the <br />EHD until December 2025. Operator shall retest the sterilizer after a failed test.. The sterilizer may not be used until a <br />assing spore test is obtained. <br />Submit spore test results to the EHD monthly until December 2025. <br />Notes: <br />1. Inspection report emailed to facility owner, Melissa Bambula-Santos, at canvas tat2(cDsbcglobal.net. <br />2. Maintenance was last performed on the autoclave on August 2017. <br />3. EHD is to approve any digital forms prior to use by the practitioners. <br />4. The 2025 — 2026 permit fees are currently due and should be paid immediately, if not paid. <br />Reinspection onlabout: A reinspection fee of $179 per hour may be charged.
