This information was received on 12/9/22, 114 days late. <br />REGULATION GUIDANCE: An operator that receives an inspection report shall have 30 days to implement the <br />corrections specified in the inspection report and comply with permit conditions. The corrective action shall include <br />all of the recommendations made in the report. The operator shall submit a written response that includes a <br />statement documenting corrective actions taken or proposing corrective actions which will be taken. <br />CORRECTIVE ACTION: Ensure that a written response documenting corrective actions taken or proposed is <br />submitted and all corrections implemented within 30 days of receiving an inspection report. <br />This is a repeat violation, Class II. <br /> 203 CCR 2712(b)(1-2) Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br />OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. Work orders <br />from the spill container replacement in December 2022 were not found on site. <br />REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br />include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br />records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs <br />of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. <br />CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br />last three years on site. Submit copies of any missing work orders and alarm records to the EHD. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 8/17/23. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br />($156).
