OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -On multiple DO reports, Section 5 reports does not include a follow-up actions to correspond to appropriate compliance issues from section 3. -Missing signatures and printed name for multiple DO reports on Section 6. -The training record for dates of"Assuming Responsibility as a Facility Employee"should not be the same as the employees. It is written as 6/13/2022 for employees and facility owners. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. OBSERVATION: The designated operator failed to check and note the correct test dates for the Secondary Containment Testing on the designated operator inspection reports.The date listed was 5/17/2021 for multiple DO reports. The correct date for Secondary Containment Testing is 4/23/2021. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class ll. 304 CCR 2631(a); HSC 25290.1(c) Failure to construct, operate, and maintain primary containment as product-tight. OBSERVATION: Liquid is observed inside the UDC 1-2 and 3-4 and on the pipe of the diesel indicating a leak in the primary containment. Per technician, Nick Janoe,the liquid is coming from the diesel pipes. REGULATION GUIDANCE:All primary containment for the UST system must be product tight. CORRECTIVE ACTION: Immediately have a properly licensed,trained, and certified contractor repair or replace the failed component(s), under permit and inspection of the EHD if necessary.
