The site map checked NA for emergency response equipment. The <br />site has multiple emergency response equipment listed under the consolidated emergency response/contingency <br />plan. Also, spill control and fire extinguishers are listed on page 2 of the site map. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: unchecked the NA box for emergency response equipment and submit the business plan. <br />This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br />OBSERVATION: The consolidated emergency response/contingency plan has incorrect and outdate information. <br />-Facility ID # listed is on page 1 of the plan is incorrect. <br />-The Alternate Emergency Coordinator, Rich Tucker listed on page 2 of the plan is no longer working for the <br />facility. Rich Tucker stopped working for the facility a couple years ago. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a <br />release or threatened release of hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan for calendar year 2020 was submitted late and incomplete on 3/16/2020. The <br />inventory and the plans were not included in the submittal.
