PAYMENT TERMS Payment is due upon delivery of services. Payment may be made by check with invoices due upon receipt. Any invoice past thirty days (30), Sharps Solutions has the option to suspend service. Returned checks are subject to a $35.00 returned check fee. LENGTH OF AGREEMENT Upon execution by both parties, this agreement shall remain in effect on a month-to-month basis. Customer or Sharps Solutions may discontinue service at any time. Pricing included herein is for 24 months from (Date of Proposal).Addition of new waste streams will be quoted as requested. OTHER TERMS Sharps Solutions may subcontract any of its collection, treatment and disposal obligations to a duly licensed and permitted medical waste services agent. Sharps Solutions may request to assign this agreement to a subsidiary or affiliate, but will not do so without prior notification to, and consent from, customer. If any term or provision of this agreement is determined to be illegal, unenforceable, or invalid in whole or in part for any reason, such illegal, unenforceable, or invalid provisions or part thereof shall be stricken from this agreement, and such provision shall not affect the legality,enforceability,or validity of the remainder of this agreement. Sharps Solutions will pick up container(s), transport to the designated medical waste treatment facility, treat and dispose of biowaste,trace-chemotherapy waste, pathological waste and pharmaceutical waste
