HSC 119315 - Invoices and log kept for disposable, pre-sterilized equipment <br />OBSERVATION: Sterilization certificates were not available for Precision Needles lot numbers 240610. <br />CORRECTIVE ACTION: When using disposable, single-use, pre-sterilized instruments, the following records must be <br />maintained: Written proof on company or laboratory letterhead showing that the pre-sterilized instruments have undergone a <br />sterilization process. Writen proof shall clearly identify the instruments sterilized by name or item number and shall identify the <br />lot or batch number of the sterilizer run. Submit the sterilization certificates for lot numbers 240610. <br /> <br />#35. HSC 119313 - Operation and employee training records present <br />OBSERVATION: The Infection Prevention Control Plan’s (IPCP) was updated and training for Leilani Smith has not been <br />completed. <br />CORRECTIVE ACTION: Submit IPCP training record for Leilani Smith to the EHD. <br /> <br /> <br /> <br /> <br />Submit evidence of corrections to the EHD <br />IMMEDIATELY <br /> <br /> <br /> <br />Notes: <br />1.
