Sterilizer shall be spore-tested after initial installation , after major repair, and at least once per month. A written sterilization shall be maintained for 3 years. The log shall include spore-test results each sterilization cycle, date, contents, exposure time and temperature, results of the Class V integrator for every cycle/load, and evidence of a acceptable spore test before reuse of the sterilizer after a failed spore-test. HSC 119315(b) — A spore test was not available for the sterilizer after the initial installation. Facility may not open until a negative spore test is submitted to the EHD. 32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. The body art facility shall be free of insect and rodent infestation . The procedure area must be separated, by a wall, ceiling to floor, from nail and hair activities and be separated from all business not related to body art. Approval of co-located body art facilites are at the discretion of the LEA, A procedure area must be equipped with adequate light to perform a procedure. Each procedure area shall have lined waste containers. HSC 119314 & 119317 — The baseboard in the blue room that is next to the sink is in need of repair. The baseboard in the yellow room next to the sink was not permanently fixed to the wall. The floor between the red room and the shop consisted of absorbent material. Repair the baseboards in both rooms and replace the absorbent flooring material used between the red room and the shop, then provide photographic evidence of correction to the EHD. Correct all violations by November 30 , 2021 and provide evidence of corrections to the EHD . Notes: 1 . Inspection report emailed to owner, Sintia Melia, at Sintia Melia sintiamejia@yahoo. com. 2. Facility may open after corrections are received by the EHD. 3. Ensure all practitioners are trained on the Infection Prevention and Control Plan . Keep training records on site for 3 years. 4. Practitioners Bloodborne Pathogen Training Certificate and the health permit should be posted in a conspicuous place. 5. The autoclave was approved by EHD. 6.
