Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Provide written notification to ust(a_sigov.org to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). <br /> a. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br /> backfilling. <br /> b. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic test (or <br /> equivalent), after concrete is set. Proper drainage must also be verified. <br /> c. Since concrete will be broken near the 91 tank, a secondary containment test must be <br /> performed on the 91 tank once concrete is poured in accordance with the manufacturer's <br /> guidelines. <br /> Please be aware that the facility cannot receive fuel in the 91 tank prior to the EHD inspection <br /> and approval. <br /> 4. Individuals installing underground storage tank system components must possess or work <br /> under the direct and personal supervision of an individual physically present at the work site <br /> who possesses all of the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board <br /> b. A valid, current UST Installation/Retrofitting certificate issued by the International Code <br /> Council (ICC) <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) being <br /> installed <br /> 1868 E.
