A body art facility shall have floors and walls that are smooth, nonabsorbent, free of open holes, and <br />washable. HSC 119314(a)(2). <br /> <br />OBSERVATIONS: <br />1. The baseboard within the procedure room had a missing piece behind the procedure bed. <br /> <br />CORRECTIVE ACTIONS: <br />1. Repair the baseboard and send proof of repair to EHD. <br /> <br /> <br /> <br />33. A body art facilty shall have all counter surfaces and service trays with smooth, durable, and <br />nonabsorbent finish. <br /> <br />OBSERVATIONS: <br />1. Procedure bed was observed covered with blankets – Corrected on site <br />2. Procedure bed (no longer in use) was being stored in the procedure room. This procedure bed had tears <br />which exposed absorbent padding. - Corrected on site <br />3. Flowers within the procedure area were observed. - Corrected on site. <br />4. Curtain that goes into the procedure area sink is absorbent. <br />5. Canvas painting is not smooth enough to be easily cleanable. <br /> <br />CORRECTIVE ACTIONS: <br />4. Remove the curtain covering the procedure sink area or replace with nonabsorbent curtains. Submit <br />corrections to EHD. <br />5. Remove the canvas painting or cover in clear coat sealant. <br /> <br /> <br />Corrections due to EHD by May 12, 2025 <br /> <br /> <br /> <br />Notes: <br />1. Report is emailed to saraprice@pricegroup.com. <br />2. Facility has not had anyone practicing body art since May 2024. Few records available for review on old forms. New client <br />consent forms were made. New copies given to EHD for review. <br />3. No pickups for sharps waste as the facilty does not get a lot of clients for body art. Noted to operator that pick ups need to <br />be done for sharps waste when the container is full or atleast once per year. Records of pick up are to be kept for a period <br />of 3 years. <br />4. Sharps Compliance is the sharps waste hauler.
