OBSERVATION: The designated operator failed to document all the alarms from the attached alarm history on multiple designated operator inspection report and failed to check that they were responded to appropriately. The missing alarms include the following sensor out alarms: *L1 (87 STP sump sensor)on 4/7/23. *L2(87 annular sensor)on 4/7/23. *L3(91 STP sump sensor)on 2/12/23, 3/23/23, 3/27/23, and 4/10/23. *L4(diesel STP sump sensor)on 2/13/23, 3/23/23,4/9/23, and 4/10/23. *L5(diesel/91 annular sensor)on 4/5/23 and 4/7/23. REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the inspection report. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -Owner/operator printed name was left blank. -The box for mechanical floats in the UDCs was not marked. -The question regarding alarms since the previous inspection was marked N/A when alarms had occurred. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. OBSERVATION: The designated operator failed to visually inspect the tank top sumps for leaks,water, debris, or other issues during the visual inspections despite there being multiple alarms. REGULATION GUIDANCE: During the inspection, the designated operator shall visually inspect all spill containers, fill pipes,fill caps, and UDC sumps, and any tank top sump that had an alarm for which there is no record of a service visit,for damage, and for the presence of leaks,water, or debris, and check that the sensors are located properly. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are inspecting all of the required areas and noting it on the on the reports.
