HSC 119314 - Handwashing facilities properly supplied and accessible, warm water <br />OBSERVATION: <br />1. The waste container in Fabian’s procedure room was not lines. <br />2. The handwash sink in the restroom does not have hot water. <br /> <br />CORRECTIVE ACTION: <br />1. This violation was corrected at the time of the inspection. Julissa Reyes lined the waste container with a trash bag. <br />2. Ensure the sink is supplied with both hot and cold running water. Notify the Environmental Health Department (EHD) <br />once the sink has been repaired and a reinspection will be conducted to verify compliance. <br /> <br />18. HSC 119303 - Client records approved and available - Consent form and questionnaire <br />OBSERVATION: The client medical questionnaire does not address requirements for antibiotics prior to surgery or dental <br />procedures. <br /> <br />CORRECTIVE ACTION: Update the medical questionnaire to ensure compliance with all items listed in HSC 119303(b). Submit <br />a copy of the revised questionnaire to the EHD. <br /> <br />33. HSC 119314 - Workstation, surfaces, including chairs, armrests, etc. in good repair <br />OBSERVATION: The counter next to Julissa’s procedure area sink does not have a smooth, durable, and nonabsorbent finish. <br /> <br />CORRECTIVE ACTION: Remove the counter from the procedure area. <br /> <br />34. HSC 119312 - Permit/registration posted <br />OBSERVATION: Practitioner permits were not posted for the following practitioners: Julissa Reyes, Fabian Reyes, & Eddy <br />Reyes Jr. <br /> <br />CORRECTIVE ACTION: Ensure all practitioners obtain and post their practitioner permits at the facility. <br /> <br />35. HSC 119313 - Operation and employee training records present <br />OBSERVATION: The Infection Prevention Control Plan (IPCP) is inaccurate. The IPCP documents the use of Madacide as <br />disinfectant, however, per Julissa they are currently using Sani-Cloth. <br /> <br />CORRECTIVE ACTION: Update the IPCP and submit the plan to the EHD. <br /> <br /> <br /> <br />
