She emailed me photos <br />taken and a report. <br />ALANIZ <br />11/19/2020 Sent an email to Henry Fontes, <hfontes@wm.com>, asking to provide an email <br />once the truck has been fixed. <br />ALANIZ <br />12/15/2020 Attempted to call Henry Fontes, Waste Management, 209-313-4814. His number <br />came back as disconnected. <br />ALANIZ <br />Looked up facility in CERS (1333 E Turner) and called the environmental contact, <br />Sam Yager, (530) 356-3756. Left a message on his phone. <br />Sent an email to Henry asking for an update. His email is <hfontes@wm.com>. <br />Received a call back from Sam. The correct number to call is 209-313-4817. It <br />was advised I call him tomorrow as it was the end of the work day. <br />12/28/2020 Have not received anything from the responsible party. Called Henry Fontes, <br />Waste Management, 209-313-4817. Left message. <br />ALANIZ <br />01/04/2021 Received an email from Eric Mankins, <EMankins@wm.com>, containing the <br />disposal record for the oily absorbent. The disposal record did not have the <br />signed TSDF copy. Sent a reply back asking for a confirmation that the truck was <br />repaired. <br />ALANIZ <br />01/06/2021 Received a reply from Eric Mankins, <EMankins@wm.com>, saying I apologize <br />for the delay I needed to confirm with the Maintenance Shop. Truck 104530 has <br />not been repaired as of yet and is out of service until repairs have been <br />completed. Filed the email with the report. <br />ALANIZ <br />02/19/2021 Received the final disposal record for the waste from Eric Mankins, <br /><EMankins@wm.com>, on 2/16/2021. Complaint can be closed. <br />ALANIZ <br />03/25/2021 An error was made where the wrong complaint number was used for a past daily. ALANIZ <br />Corrected it. <br />Discussed abating complaint with Elena Manzo, lead senior. An emergency <br />response record needs to be made for her site visit. <br />03/25/2021 Sent an email to Eric Mankins, <EMankins@wm.com>, asking if the truck was <br />fixed. He replied that it was still out of service. <br />ALANIZ <br />Began putting the complaint together to abate it. <br />04/15/2021 Received a call from Ashan Williams (RP representative) concerning billing, <br />clarified billing information. <br />MANZO <br />05/13/2021 Need ER record to abate complaint. Discussed with Elena Manzo, lead senior. ALANIZ <br />05/08/2023 Gathered documents and abated complaint.
