<br />4The PSM Program Implementer may spot check to verify that the contractor has <br />trained their employees in PSM Program elements by reviewing the written <br />program, observing employees as they work, or questioning employees as to <br />their knowledge of the programs. <br /> <br />The contractor is specifically responsible for the following training: <br /> <br />· The contractor must make sure that each contract employee is instructed in <br />the known potential fire, explosion, or toxic release hazards related to his or <br />her job and the process, and in the applicable provisions of the emergency <br />response plan as provided by the facility. <br /> <br />· The contractor must document that contract employees have received and <br />understand the training received. The record must include the identity of the <br />contractor's employee, the date of training, and the means used to verify that <br />the employee understood the training. <br /> <br />4. CONTRACTOR CERTIFICATION OF HAZARD COMMUNICATION <br /> <br />The undersigned Contractor Representative has been instructed on the <br />hazardous chemicals their employees could be exposed to while working at the <br />Leprino Foods - Tracy facility. <br /> <br />· Contractor has reviewed the Leprino Foods - Tracy evacuation plan and <br />accepts full responsibility for implementing an evacuation plan for all <br />employees under his/her direction to safely evacuate their work station while <br />working at the Leprino Foods - Tracy facility. <br /> <br />· Contractor has informed all its employees of all hazardous chemicals they <br />could be exposed to and which measures should be taken to lessen the <br />exposure of these chemicals while working at the Leprino Foods - Tracy <br />facility. <br /> <br />· Contractor agrees to both notify the Leprino Foods - Tracy Process Safety <br />Management (PSM) Program Implementer and train all new hires prior to <br />introducing them to any existing or newly formed working groups. <br /> <br />· Contractor further agrees to provide and ensure the same level of <br />information/training to all sub-contractors they deem necessary to employ in <br />order to fulfill the scope of work that has been negotiated/contracted with <br />Leprino Foods - Tracy. <br /> <br />· Contractor will identify all sub-contractors and their employees to the Process <br />Safety Management (PSM) Program Implementer. <br /> <br />Initials of Contractor Representative: __________
