Complaint Investigation Summary Report # 5105 <br />COMPLAINT ID: COHONB005 Site Location: 25501 S GATEWAY BLVD <br />Activity Date <br />Activity Summary <br />Recorded by <br />02/01/2022 APPROXIMATELY 9:50 PM ON FRIDAY, AN AMMONIAALARM WAS ERNEST <br />TRIGGERED FOR A 30 PPM READING (FROM ALARM HISTORY). NO <br />NOTIFICATION WAS SENT WAS BY THE ON-SITE NOTIFICATION SYSTEMS. <br />SHIFT ENDED FOR COSTCO EMPLOYEES AT 3:30 PM ON FRIDAY. WHEN <br />THE OPENING MANAGER ARRIVED AT THE FRONT OFFICE AT 3:17 AM ON <br />SATURDAY, HE OBSERVED THE AMMONIAALARM WAS TRIGGERED. HE <br />CALLED APCCO, CONTRACTED REFRIGERATION COMPANY FOR ONSITE <br />RESPONSE. EMPLOYEES ARRIVED APPROXIMATELY 4:00AM FOR <br />FOLLOWING SHIFT. EMPLOYEES WERE TOLD TO STAY IN THEIR <br />VEHICLES. APCCO ARRIVES AT 5:00AM FOR ASSESSMENT. ASSESSMENT <br />IS MADE AND A VALVE ON EVAPORATOR 37 WAS FOUND TO BE THE <br />ISSUE. EVAPORATOR 37 WAS ISOLATED AND PUMPED DOWN. 3% OF <br />RESIDUAL AMMONIA REMAINS INSIDE EVAPORATOR 37. FIRE <br />DEPARTMENT WAS NOTIFIED AND ARRIVE ON SCENE AT <br />APPROXIMATELY 7:40AM. ALL CLEAR WAS GIVEN AT APPROXIMATELY <br />7:40AM. VERISK3E, COMPLIANCE COMPANY, WAS NOTIFIED AROUND <br />10:00AM. ON CALL PERSON FROM SAN JOAQUIN COUNTY <br />ENVIRONMENTAL HEALTH WAS NOTIFIED AT 10:44. COSTCO <br />APPROXIMATES 1 POUND OF AMMONIA WAS RELEASED. NO EXPOSURE <br />OR INJURIES OCCURRED DURING THE INCIDENT. <br />Completed notice to abate. <br />Sent email to Jimmee Kim D172FAC@costco.com ccd. Haza Saeed EHD. <br />02/02/2022 Needs to be deleted. ERNEST <br />02/10/2023 Read through narratives. Generated notice to abate from Frederick Ernest's daily <br />serial number. Frederick Ernest is no longer with EHD. To continue review another <br />day. <br />PRESTO <br />02/13/2023 Reviewed notice to abate, corrective actions needed investigation report, <br />documentation that alarms are being verified for functionality and statement that <br />PRESTO <br />CUPA will be notified for future hazardous materials releases. No information in <br />regular facility narratives that information requested was received. No information <br />in complaint narratives that information has been received.
