SAN JOAQUIN <br />Cou N T V - <br />alSrer Greotnesi groe.s here <br />Environmental Health Department <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: LOVES TRAVEL STOPS AND COUNTRY STORES Date: 05/04/2023 <br />Address: 15250 N THORNTON RD RD, LODI <br />Owner/Operator JESSE DIAZ Telephone: (405) 242-2455 <br />Program Element: 2546 - RELEASE/SPILL RESPONSE (EXCLUDING JOINT TEAM) Complaint #: COTNXR0U4 <br />Responsible Party Contact: JESSE DIAZ <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />EHD received notification that on 05/4/202 at 1855 during fueling, a customer overfilled a gas tank and approximately 5 <br />gallons of gasoline was released onto the asphalt at 15250 North Thornton Rd Lodi. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />COMMENTS AND OBSERVATIONS <br />On 05/04/2023 a customer overfilled at gas station pump #16 and caused a release of approximately 5 gallon of gasoline onto <br />asphalt. Per telephone conversation with Jenny Lo (209 333 9392) on site Store manager, facility employees conducted clean <br />up of gasoline spill using kitty litter and placed the waste in a 55 gallon drum and stored at facility hazardous waste storage <br />area. <br />Make a hazardous waste determination for all waste materials and dispose of them as required. Provide copies of disposal <br />records of any waste generated from cleanup operation to EHD. <br />Also make sure pump #16 is verified that no gasoline went onto the under dispenser containment (UDC) sump. Please be <br />advised that the disposal of any waste [HSC 5411] or disposal of any hazardous waste, or the causing therefor, is prohibited <br />when the disposal is at a facility that is not permitted or authorized for hazardous waste disposal.[HSC25189.5(a)]. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
