HSC 119314 - Sharps containers labeled, used, and disposed of appropriately <br />OBSERVATION: The procedure area did not have a sharps waste container. Per Belen, she had it picked up recently and is still <br />waiting for her new sharps waste container to arrive. <br />CORRECTIVE ACTION: Place a sharps waste container in the procedure area. A sharps waste container shall meet all the <br />requirements listed in HSC 119314 (e). <br /> <br />#11. HSC 119314 - Handwashing facilities properly supplied and accessible, warm water <br />OBSERVATION: The handwash sink in the procedure area was not supplied with containerized liquid soap. <br />CORRECTIVE ACTION: Supply the procedure area handwash sink with containerized liquid soap. Submit photos to the <br />Environmental Health Department (EHD). <br /> <br />#18. HSC 119303 - Client records approved and available - Consent form and questionnaire <br />OBSERVATION: The medical questionnaire does not ask about all current medications. <br />CORRECTIVE ACTION: Update the medical questionnaire to include information regarding client medications and submit a <br />copy to the EHD. <br /> <br />#35. HSC 119313 - Operation and employee training records present <br />OBSERVATION: The Infection Prevention Control Plan (IPCP) includes disinfection procedures that are not consistent with the <br />Caviwipe disinfection directions listed on the label. <br />CORRECTIVE ACTION: Update the IPCP to ensure proper disinfection procedures per the manufacturer’s guidelines and <br />submit the revised IPCP to the EHD. <br /> <br /> Submit evidence of corrections to the EHD by <br />04/30/2025. <br /> <br /> <br />NOTES: <br />1. The inspection report was emailed to the facility owner Belen Diaz, belensbrowrm@gmail.com <br />2. Reviewed the client consent forms, medical questionnaire, and after-care instructions. <br />3. Reviewed the IPCP. Ensure to provide training annually and keep records for 3 years. <br />4. Reviewed sterilization certification for Beauty Angels Academy needle lot #BAA082021. <br />5. Sharps waste disposal is provided by Sharps Compliance. Ensure to keep disposal records for 3 years. <br />6. Per Belen, she has not performed any body art procedures since the beginning of 2023. Belen states she anticipates to <br />start again once she has paid and received her permits. <br />7. The facility and practitioner has unpaid permit fees. Immediately pay the outstanding balance. <br />a. HSC 119312: A body art facility shall not conduct business without a valid health permit. <br />b. HSC 119306: (a) A person shall not perform body art at any location other than a permitted permanent or <br />temporary body art facility. (b) A person shall not perform body art if he or she is not registered with the local <br />enforcement agency. <br />
