SANAAQUIN <br /> Environmental Health Department <br />LOU N T Y <br />Time In: 12:40 pm <br />Time Out: 1:10 pm <br />COMPLAINT INSPECTION REPORT <br />Name of Facility PHILLIPS FARMS Date 06/21/2023 <br />Address: 4580W HVVY 12 , LODI 95240 <br />Owner/Operator: PHILLIPS, DONALD Telephone: (209) 368-7384 <br />Program Element: 1600- FOOD PROGRAM Complaint #: C00055910 <br />Responsible Party Contact <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />ON 6/12/2023 AT 12:30PM COMPLAINANT ATE AT THE FACILITY AND ORDERED A BACON LETTUCE TOMATO <br />SANDWICH WITH POTATO SALAD, MIXED BERRY PIE, AND ICED TEA. THAT AFTERNOON STARTED NOT FEELING <br />VERY WELL AND BY EVENING COMPLAINANT WAS HAVING STOMACH PRESSURE, NAUSEA, VOMITING, & <br />DIARRHEA. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />#6 Handwashing Facilities Supplied and Accessible <br />OBSERVATIONS: The cafe warewash area hand sink was blocked by a fan. Keep hand sinks accessible at all times when <br />operating. Corrected on site. <br />CAL CODE DESCRIPTION: Handwashing soap and towels or drying device shall be provided in dispensers dispensers shall be <br />maintained in good repair. (113953.2) Adequate facilities shall be provided for hand washing, food preparation and the washing of <br />utensils and equipment. (113953, 113953.1, 114067(f)) <br />#7 Hot and Cold Holding Temperatures <br />OBSERVATIONS: Observed egg container on bakery counter, per employee it was there for approx. 1 hour and was used <br />for another item earlier. Keep shell eggs at 45 F or below if they are not actively being prepped. Corrected on site, eggs <br />were placed in cooler. <br />The potato salad at the cook line ice bath was 41-43 F. The container was not fully submerged in the ice. Keep PHFs fully <br />submerged in ice and maintain ice level to keep PHFs at 41 F or below when using ice bath or provide refrigeration. Correct <br />today. <br />CALCODE DESCRIPTION: Potentially hazardous foods shall be held at or below 41/ 45°F or at or above 135°F.
