Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: C00054834 Site Location: SOUTH BOUND 5 AT ROTH ROAD <br />Activity Summary <br />Activity Date <br />04/21/2022 CALOES# 22-2284 <br />Received page at approximately 3PM. <br />OES report indicates: <br />A semi-truck jack-knifed on the freeway, puncturing the <br />saddle tank and causing diesel to release onto the dirt embankment on the right <br />hand shoulder. The tank has a very small ongoing leak due to the tank being <br />inaccessible at this time. CHP has closed 1 lane of the southbound <br />1-5 with two additional lanes remaining open. Cal Trans is on scene for cleanup. <br />No waterways impacted. <br />Incident occurred on Southbound 15 just North of Roth Rd. <br />a. SUBSTANCE: / b.QTY: / Amount / Measure / c. Traffic was heavy on 15 due to incident. <br />On site I meet CHP Officer Scott Siria who indicated the driver of the semi truck <br />lost control of the vehicle due to rain and crashed into the ditch in front of the <br />southbound roth road exit. The semi truck jackknifed which caused the saddle <br />tank to rupture. The diesel leaked onto the soil. <br />I discussed cleanup with Juan Armendarez, District HAZMAT manager for <br />Caltrans. He indicated the responsible party has not contracted a clean up <br />company but he indicated Ancon environmental seem to be the party contracted. <br />Juan A. also indicated he will tarp the affected soil. I noted the responsible parties <br />information as Oasisexpressinc1@gmail.com 916 243 0201, at 5327 Chatswood <br />Way Antelope Ca 95843. <br />The truck was removed from the ditch, and Scott S. indicated he would increase <br />the amount of diesel to 90 gallons released due to comments from the driver, <br />indicating the saddle tank was full of diesel. <br />I traveled back to the office and passed off information to John Alaniz for follow <br />up. <br />04/21/2022 EHD received CalOES spill report 22-2284 on 4/21/2022. 90 gallons of diesel fuel <br />were released from a truck accident onto soil. No storm drains or waterways were <br />impacted. Ancon Services was hired by the responsible party to conduct clean up. <br />Site was marked for USA dig and covered with a tarp. <br />Notice to Abate was emailed to Oasis Express at oasisexpressinc1@gmail.com. <br />Recorded by <br />ERNEST <br />GARCIA-MEJ1A <br />05/05/2022 Called Oasis Express, left a message asking for an update on the spill on 15 near ERNEST <br />roth road exit. <br />Sent email <br />06/06/2022 Received email from Armendarez, Juan J@DOT <juan.armendarez@dot.ca.gov > ERNEST <br />indicating clean up will occur at 9am on wednesday 6/8/2022.
