Environmental Health Department <br /> Time In: 10:40 am <br /> Time Out: 10:55 am <br /> COMPLAINT INSPECTION REPORT <br /> Name of Facility: TACO BELL#35901 Date: 08/15/2023 <br /> Address: 2380 W KETTLEMAN LN, LODI 95242 <br /> Owner/Operator: BRENT FLYNN Telephone: (209)369-3359 <br /> Program Element: 1600- FOOD PROGRAM Complaint#: C00056048 <br /> Responsible Party Contact: <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT STATES THAT ON 08/04/2023 SHE ORDERED THE SHREDDED BEEF GRILLED CHEESE DIPPING <br /> TACO CRAVING BOX; INCLUDED A SHREDDED BEEF GRILLED CHEESE DIPPING TACO, NACHO CHEESE SAUCE, <br /> AVOCADO RANCH SAUCE, CHALUPA SUPREME, BEEFY 5 LAYER BURRITO, &CHIPS WITH NACHO CHEESE <br /> SAUCE. COMPLAINANT STATES THAT WITHIN 45 MIN OF EATING SHE BEGAN FEELING SYMPTOMS OF SEVER <br /> CRAMPING AND DIARRHEA. COMPLAINANT STATES THAT NO DOCTOR WAS SEEN BUT SHE DID CALL THE <br /> ADVICE NURSE, AND SHE IS STILL FEELING SYMPTOMS THOUGH THEY HAVE SUBSIDED SLIGHTLY. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Manager states numerous other orders of the taco craving box ordered <br /> during the last couple of weeks and no similar complaints have been received.
