The following is an itemized list of underground storage tank violations that have not <br /> been addressed for LODI MEMORIAL HOSPITAL as of December04, 2023. <br /> Open violations from September 18, 2023 inspection <br /> Violation#104- Failure to submit a complete and accurate UST Monitoring Plan. <br /> OBSERVATION The monitoring plan is not current. The people listed as having responsibility(Tim Duncan &Jim <br /> McCarthy) are no longer with Lodi Memorial Hospital. <br /> REGULATION GUIDANCE: The monitoring plan must be uploaded to the California Environmental Reporting <br /> System (CERS). <br /> CORRECTIVE ACTION: Immediately log into CERS, make the necessary changes, and submit for review by the <br /> EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #105 -Failure to submit a complete and accurate UST Response Plan. <br /> OBSERVATION The response plan is not current. The most recent <br /> certificate of financial responsibility in CERS is dated 10/19/2020 and the most recent letter of chief financial officer <br /> is dated 10/27/2020. <br /> REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the required documents, and maintain a copy on site. <br /> Page 1 of 3 <br />
