Please <br /> provide the following additional information and address the following comments: <br /> 1. Provide current contractor's training certification for installation and testing of all proposed UST <br /> equipment/components <br /> a. Clarify which flex connector will be used & provide contractor's training certification <br /> 2. Provide manufacturer's cut sheets for proposed UST system components, including the following <br /> components listed in the parts list and application: <br /> a. Leak detectors <br /> b. Flex connectors <br /> c. Mechanical overfill flappers <br /> d. Sensors <br /> e. Monitoring panel <br /> f. Tanks <br /> g. Piping <br /> h. Spill Containers <br /> i. Sumps <br /> j. Vent Sumps <br /> 3. The cut sheet submitted for the spill container is for the OPW 1-2100-EVR series. Page 7 of 8 section G of the <br /> application lists the spill container as the OPW 411-5. Clarify the accurate spill container being used and update <br /> the paperwork as needed. <br /> 4. Page 3 of 8 of the application— <br /> a. No workers compensation number is listed. <br /> b. No fire permit information is listed. <br /> 5. Page 8 of 8 of the application— <br /> a. Section I lists the sump sensor as model as V/R330020-536. This is not a sensor model <br /> number. Resubmit with accurate sensor information. <br /> b. Section lists does not have model number. Resubmit with accurate dispenser pans information. <br /> c. Section lists the line leak detector model as V/R332250-001. This is not a line leak detector model <br /> number. Resubmit with accurate line leak detector information. <br /> d. Section J — Under Dispenser Containment(UDC) Composition lists both fiberglass and steel. Clarify the <br /> accurate composition of the UDCs being used and update the application as needed. <br /> 6. Page FS1.1 General Tank & Line Notes #2 list the 87 tank as 15,000 but the application page 3 of 8 list <br /> 20,000 gallon tank. Resubmit with clarification. <br /> a. FS1.1 & FS1.2 Map of underground tanks need clarification. Which fuel tank will be the end <br /> tank? It does not match page FS3.0 <br /> 2 <br />
