SAN ) OAQUIN Environmental Health Department <br /> WASTE DISCHARGE/SPILL RESPONSE <br /> NOTICE TO ABATE <br /> Responsible Party: PILOT TRAVEL CENTERS LLC Date: Jun 19, 2018 <br /> RP Mailing Address: 5508 LONAS RD Record ID#; C00046943 <br /> Knoxville, TN 37909- Program Element 2546 <br /> Spill Location: 15100 THORNTON RD, LODI Release/Spill Response (excluding Joint Team) <br /> NATURE OF WASTE DISCHARGEIWASTE RELEASE: <br /> Approximately 5 gallons of waste oil spilled out of a truck into the pavement and into a storm drain that has a catch basin. <br /> Cause is unkown. The release is stopped and contained. A storm drain leading into an onsite retention pond was impacted <br /> and they were able to recover the release. <br /> OBSERVATIONS: <br /> Observed what appeared to be the residuals of absorbent starting from a parking space and leading into a storm drain. The <br /> substance cleaned up is believed to be used motor oil. The impacted area on the asphalt had been cleaned by facility <br /> personnel. Ponder Environmental opened storm drains to try and asses the extent of contamination. The storm drain directly <br /> west of where the oil entered the the drain system showed signs of sheen. The second storm west of that did not show any <br /> signs of contamination. A storm drain north of where the oil entered did not show signs of contamination. Per Ponder <br /> Environmental, the affected surface is to be power washed and the affected line of the storm drain is to be jetted. <br /> VIOLATIONS: <br /> • Discharge of any waste, including sewage <br /> (CA Health and Safety Code 5411) <br /> ° Discharge of waste or sewage to water or within 150 ft. of waterway <br /> (SJC Ordinance Code Sections 9-1125.3, 9-1125.4) <br /> • Improper disposal of a hazardous waste <br /> (CA Health and Safety Code 25189.5) <br /> Permitting or Maintaining a Nuisance <br /> (SJC Ordinance Code Section 8-5100) <br /> ° Other Violation <br /> CORRECTIVE ACTIONS I ORDER: <br /> • Investigate and abate contamination under the oversite of the appropriate lead agency. <br /> • Submit copies of hazardous waste manifests/disposal records to the EHD. <br /> Within 30 days. <br /> ° Other Correction <br /> EHD 25-01 Rev.08/17/17 Page 1 of 2 WASTE DISCHARGE/SPILL RESPONSE NOTICE TO ABATE <br /> 1868 E.
