Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />August 29, 2023 <br /> Facility Address: <br /> 1777 W YOSEMITE AVE, MANTECA <br /> Facility Name: <br /> KAISER FOUNDATION - MANTECA <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />Observations 5,472 cubic feet of nitrogen,1200 cubic feet of medical air, and 55 gallons of propane were found on <br />site that have not been reported to CERS. <br />OBSERVATION: The Hazardous Materials Inventory forms for nitrous oxide, liquid oxygen, carbon dioxide were not <br />complete. The unit of measure and max daily amounts were incorrect. The max daily amount for liquid oxygen is <br />listed at 200,000 cubic feet but needs to be listed at 6,525 gallons. arbon dioxide max daily amount is listed at <br />1,000 cubic feet but should be listed at 200 pounds. nitrous oxide max daily amount is listed at 2,500 cubic feet but <br />should be listed at 1,050 pounds. <br />REGULATION GUIDANCE: Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br />approval. <br />OBSERVATION: The Hazardous Materials Inventory forms for [NOTE WHICH CHEMICALS] were not complete. <br />[NOTE WHAT WAS LACKING OR INCOMPLETE] . <br />REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br />of the business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: A site map was not complete. The site map is missing entrance and exits to all of the buildings. <br />Emergency response equipment ( fire extinguisher, eye wash, safety shower). The site map should be for each floor <br />of the buildings that are multiple stories. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br />FA0003978 PR0519899 SC001 08/29/2023 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 2 of 4 <br />1868 E.
