HSC 119315-Invoices and log kept for disposable, pre-sterilized equipment <br /> OBSERVATIONS: <br /> Sterilization certificates were not available for the Emalla cartridges with the lot#07/2023 and#08/2022. <br /> CORRECTIVE ACTIONS: <br /> A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and client, <br /> date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br /> sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br /> minimum of 90 days after use. Provide EHD with sterilization certificates for the instruments mentioned above. <br /> Correct all violations and provide evidence of correction to the EHD <br /> by April 14, 2026. <br /> Notes: <br /> 1. Inspection report emailed to facility owner, Nicole Barber, at changesbeautelounge(cD-gmail.com. <br /> 2. Sharps waste has not been disposed of since the facility opened. Sharps waste shall be disposed of when container is full or <br /> once a year(minimum). <br /> 3. Post permit in a conspicuous place. <br /> 4. Sterilization certificates were provided for the piercing needles. <br /> 5. The Infection Prevention and Control Plan was on site. Facility owner should clean up the IPCP to prevent confusion. <br /> 6. Per facility owner, she only had one client since the last inspection.
