At the completion of the procedure, the practitioner shall provide post procedure instructions that include all the <br />information in HSC119303(a)(5). <br /> <br />OBSERVATIONS: <br />1. The post procedure instructions provided for piercing was missing, in the signs and symptoms of infection, the statement <br />“red streaks going from the procedure site towards the heart” <br />2. There were no post procedure instructions provided for the tattoo portion of the facility. <br /> <br />CORRECTIVE ACTIONS: <br />1. For the piercing post procedure instructions please include in the signs and symptoms of infection “red streaks going <br />from the procedure site towards the heart”. <br />2. Please provide post procedure instructions for permanent makeup that include the following elements: <br />a. Information on the care of the procedure site <br />b. Restrictions on physical activities such as bathing, recreational water activities, gardening, or contact with <br />animals and the duration of the restrictions <br />c. Signs and symptoms of infection <br />d. Indications of when to seek medical care <br />3. Please provide proof of correction to EHD. <br /> <br />28. The practitioner shall maintain a clean and sanitary environment. Sterile instrument packs shall be evaluated <br />before use, and if the integrity of a pack is compromised in any way, including but not limited to, being torn, <br />punctured, wet, or having evidence of potential moisture contamination, the instrument pack shall be discarded <br />or reprocessed before use. HSC 119309(a) & (h) <br /> <br />OBSERVATIONS: <br />1. Piercing needle pack with the lot number 210902 expired in September 2024 – CORRECTED ON SITE <br /> <br />33. A body art facility shall have all counter surfaces and treys shall have a smooth, durable, and non-absorbent finish. <br />HSC 119314(b)(6). <br /> <br />OBSERVATIONS: <br />1. Stuffed animal on top of the shelf within the procedure room – CORRECTED ON SITE <br />2. Inspection report was emailed to changesbeautelounge@gmail.com <br />2. BBP on Premiere’s website shows a completed course, but a mistakenly dated certificate was provided. <br />3. Piercing needle packs observed on site to be expired were removed from the facility. <br />4. Only reviewed one client consent form for piercing. No client consent forms for tattooing w ere reviewed. Practitioner <br />noted that they have not completed any permanent make up procedures. Practitioner noted only one piercing procedure <br />since opening in 2023. <br />5. No manifests for sharps waste disposal were available due to the container not being full. Noted to operator that pickups <br />for sharps waste needs to occur at a minimum of 1 time per year. Facility uses Republic Sharps for mail back system. <br />6. Reviewed invoices for purchased jewelry noting approved material. <br />
