COMPLAINT INSPECTION REPORT <br />12:35 pm <br />12:10 pm <br />Time Out: <br />Time In: <br />Environmental Health Department <br />Name of Facility: MCDONALD'S #35025 Date: 06/23/2023 <br />Address: 1360 E YOSEMITE AVE, MANTECA 95336 <br />Owner/Operator: Telephone: (209) 665-4721 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: CO0055919 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT ATE A CHEESEBURGER DURING LUNCH ON 6-13-23 AND FOUND A PIECE OF GLASS IN IT. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />No Temperature Data Collected <br />FOOD ITEM -- LOCATION -- TEMP º F -- COMMENTS <br />COMMENTS AND OBSERVATIONS <br />Complaint inspection. <br />As per manager, she is aware of the complaint and corrective measures were taken. <br />( A little piece of plastic from the salt shaker had fallen on the beef patty, went unnoticed, during cooking process and served <br />to customer). <br />During my inspection, I don't see any broken glasses at the cooking and preping areas. All light boxes covers at these areas, <br />are intact. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
