The correct date for Overfill Prevention Equipment testing is 9/23/2021 but multiple designated operator reports list 12/16/2021. The correct date for the Secondary Containment testing is 8/5/2020 but multiple designated operator reports list 8/4/2020. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. --------------------------------------------------------------------------------------------------------------------------- 2)OBSERVATION:The designated operator failed to document all the alarms from the attached alarm history on the 10/20/2023 designated operator inspection report and failed to check that they were responded to appropriately. The missing alarms include: dispenser 5/6 fuel alarm that occurred on 10/12/2023.The designated operator report on 10/20/2023 (the next report after the alarm)does not have any information written in Section 3 regarding this fuel alarm. Section 3 states"No compliance issues. SB 989 test is over due." REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the inspection report. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. ---------------------------------------------------------------------------------------------------------------------------- 3)OBSERVATION: Multiple designated operator reports are signed but do not have any follow-up actions written in Section 5 in response to the designated operator's comment that"SB 989 test is over due." REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed, signed and have follow-up actions written in Section 5(when necessary)within the required time frame in the future. This is a repeat violation, Class II.
