SAN J 0 A Q I I I Irl Environmental Health Department <br /> CCUN r <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> LODI USD-MAINTENANCE OPERATIONS#2 880 N GUILD AVE, LODI November 29, 2023 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The following hazardous materials were found on site that have not been reported to CERS: <br /> -18 boxes of hand sanitizer(12 L/box)observed in equipment storage <br /> -Several pallets of Clorox Total 360 Disinfectant Cleaner were observed in the Northeast corner of the facility <br /> -20 5-gallon buckets of Acrylic Elastomeric was observed in the container located North of the filter storage <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. The following elements were missing from the site map: <br /> -Loading areas <br /> -Internal roads (if the facility has no internal roads, list it on the map and label as N/A) <br /> -Locations of all hazardous materials <br /> -Locations of all emergency response equipment(spill kits) <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> FA0026652 PR0547024 SCO01 11/29/2023 <br /> EHD 19-01 Rev.12/06/2021 Page 2 of 3 Hazardous Materials Program OIR <br /> 1868 E.
