The site map is missing a key or legend to define PL. Emergency shut offs and utilities shut off <br />locations are not on the site map for (power, water, natural gas, generators, and chillers). Storm drain location are <br />not on the site map. Emergency response equipment ( fire extinguishers) is not on the site map. The facility is a <br />multi floor building and should have a map for each floor and needs to identify the oxygen storage room on each <br />floor. Entrances and exit need to be identified on the site map. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br />OBSERVATION: The business plan information has not been reviewed and resubmitted in the California <br />Environmental Reporting System (CERS) annually by the due date, January 15. <br />The business plan was submitted late on 10/26/2017 for the year of 2017 and was incomplete. The facility section <br />was the only section submitted. The inventory and plan section was not submitted for the year of 2017. The <br />business plan was submitted late on 11/15/2018 for the year of 2018 and was incomplete. The facility section was <br />the only section submitted. The inventory and plan section was not submitted for the year of 2018. The business <br />plan was submitted late on 10/14/2019 for the year of 2019 and was incomplete. The facility section was the only <br />section submitted. The inventory and plan section was not submitted for the year of 2019. The business plan was <br />submitted late on 7/24/2020 for the year of 2020. The business plan was submitted late on 9/29/2021 for the year of <br />2021.
