Environmental Health Department <br />CP22 - UST FULL Inspection Report <br />Date: <br />October 29, 2024 <br /> Facility Address: <br /> 7700 MORELAND ST, STOCKTON <br /> Facility Name: <br /> GAVINKO PETROLEUM* <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />REGULATION GUIDANCE: The owner or operator shall submit a "Designated Underground Storage Tank Operator <br />Identification Form" no later than 30 days after any change in the designated UST operator (s). <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br />This is a repeat violation (Class 2) Violation. <br />CA Code of Regulations 2638, 2641(j), 2715(f) 2030002 - Leak detection equipment tested by qualified service <br />technician every 12 months (TCR 9d). <br />OBSERVATION: Annual monitoring system certification and leak detector testing were previously performed on <br />8/15/2023 and was completed on 10/29/2024, 2 months past due. <br />REGULATION GUIDANCE: These tests are required once every 12 months. <br />CORRECTIVE ACTION: Take all necessary precautions to ensure testing is performed in a timely manner . <br />This is (Class 2) Violation. <br /> 9 <br />CA Code of Regulations 2638(c), 2638(d) 2030074 - Annual Monitoring System Certification Form to the CUPA within <br />30 days of completion of the test. <br />OBSERVATION: A monitoring system certification and leak detector testing was performed on 8/15/2024. A test <br />report has not been submitted to the EHD or the California Environmental Reporting System (CERS). <br />- Test report was seen on site during inspection , but has not been received by EHD <br />REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the test. <br />CORRECTIVE ACTION: Immediately provide this report to the EHD. <br />This is a repeat violation (Class 2) Violation. <br /> 10 <br />CA Code of Regulations 2637.1(d), 2637.1(e) 2010017 - Spill bucket testing results submitted to the UPA within 30 <br />days of testing. <br />OBSERVATION: Spill container testing was performed on 8/15/2023. A test report has not been submitted to the EHD <br />or the California Environmental Reporting System (CERS). <br />- test report was seen on site at time of inspection but has not been received by EHD. <br />REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the test. <br />CORRECTIVE ACTION: Immediately provide this report to the EHD. <br />This is (Minor) Violation. <br /> 13 <br />CA Code of Regulations 2637.2(d&e), 2665(a&b) 2010018 - Overfill prevention equipment inspection report form <br />submitted to the UPA within 30 days. <br />OBSERVATION: An overfill prevention equipment inspection was performed on 2/1/2024.
